(Flo Premium + Telehealth subscription)

Flo Premium + Telehealth subscription contains two components:

  • regular Flo subscription;
  • subscription to telehealth services supplied by an independent organization – HealthTap, Inc.

Regular Flo subscription. Regular Flo subscription is subject to the Flo Health Terms of Use. Please read them carefully before purchasing the Flo Premium + Telehealth subscription.

Telehealth services. Telehealth services is an additional component of the Flo Premium + Telehealth subscription. Telehealth services are supplied by HealthTap, Inc. (“HealthTap”), an independent telehealth provider acting in accordance with its own terms and policies. The use of telehealth services is subject to the HealthTap Terms of Use. Please read them carefully before purchasing the Flo Premium + Telehealth subscription.

How does it work? By purchasing the Flo Premium + Telehealth subscription you get prepaid access to the HealthTap platform. When tapping “Talk to a doctor” tab inside the App you are redirected to the HealthTap platform via UTM link that the platform identifies as the one coming from the App. Such link does not recognize you personally and is generic for users purchasing the Flo Premium + Telehealth subscription. After completing a separate registration process on HealthTap, you will be able to utilize telehealth services in accordance with HealthTap terms and policies.

 

DISCLAIMER (TELEHEALTH SERVICES)

Territorial limitations. Telehealth services are available and are intended for U.S. USERS ONLY. If you are not based in the U.S., you should cancel and stop using the Flo Premium + Telehealth subscription immediately. If you subscribe to Tier 2 telehealth services outside the U.S., you will be responsible for compliance with all local laws.

No collection or sharing of PII. Flo Health does not collect nor process any Personally Identifiable Information (PII) or Personal Health Information (PHI) on behalf of HealthTap. Flo Health does not share any of your PII or health data collected through the App with HealthTap or any of its healthcare providers, partners, agents, doctors, or representatives.

Medical Advice Disclaimer. Any information made available through the App is not intended or implied to be a substitute for professional medical advice, diagnosis or treatment. All content, including text, graphics, images and information, contained on or available through the App is for general information purposes only. Flo Health makes no representation and assumes no responsibility for the accuracy of information contained on or available through the App, and such information is subject to change without notice. You are encouraged to confirm any information obtained from or through the App with other sources and review all information regarding any medical condition or treatment with your physician. NEVER DISREGARD PROFESSIONAL MEDICAL ADVICE OR DELAY SEEKING MEDICAL TREATMENT BECAUSE OF SOMETHING YOU HAVE READ ON OR ACCESSED THROUGH THE APP.

NO HEALTH CARE PROVIDER/ PATIENT RELATIONSHIP IS CREATED BETWEEN FLO HEALTH AND AN INDIVIDUAL WHEN AN INDIVIDUAL USE THE APP. FLO HEALTH DOES NOT PROVIDE ANY REPRESENTATIONS OR WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, IN CONNECTION TO YOUR USE OF TELEHEALTH SERVICES PROVIDED BY HEALTHTAP, ALL OF WHICH ARE EXPRESSLY DISCLAIMED, SAVE TO THE EXTENT REQUIRED BY LAW. FLO HEALTH IS NEITHER RESPONSIBLE FOR NOR LIABLE FOR ANY ADVICE, COURSE OF TREATMENT, DIAGNOSIS OR ANY OTHER THIRD PARTY INFORMATION, SITE OR PRODUCTS THAT YOU OBTAIN THROUGH THE APP.

Information about HealthTap and its telehealth services, as well as inclusion of HealthTap services into the Flo Premium + Telehealth subscription, should not be viewed as recommendation, endorsement, or guarantee of any kind. Flo Health does not endorse, recommend, or make any representation about the efficacy, appropriateness, or suitability of any specific tests, physicians, doctors, medications, opinions, products or procedures that are offered through the App. Flo Health neither guarantees quality of services received via HealthTap and their conformity with applicable laws, regulations, codes of conduct, and other rules. 

You agree and acknowledge that you rely on any healthcare providers or information provided by the HealthTap via the App solely at your own risk and you assume full responsibility for all risk associated herewith. You should contact HealthTap directly should you have any questions, problems or concerns related to telehealth services rendered by HealthTap and its healthcare providers.

Limitation of liability. Flo Health is neither acting on HealthTap’s behalf as a business associate or business associate subcontractor, nor does Flo Health render any telehealth services. HealthTap and its healthcare providers are not agents, employees, or affiliates of Flo Health. All healthcare services are provided solely by HealthTap or its partner entities. 

IN NO EVENT SHALL FLO HEALTH, ITS OFFICERS, DIRECTORS, AGENTS, AFFILIATES, EMPLOYEES BE LIABLE FOR ANY INDIRECT, SPECIAL, INCIDENTAL, CONSEQUENTIAL OR PUNITIVE DAMAGES ARISING OUT OF OR IN ANY WAY CONNECTED WITH THE USE OF TELEHEALTH SERVICES PROVIDED BY HEALTHTAP.

Rules of Payment

Flo Health offers monthly subscription option for the mobile application that grant you access to HealthTap telehealth services subject to the HealthTap’s Terms of Use. Payment will be charged to your credit/debit card through your iTunes Account after you choose and confirm your purchase. Paid subscriptions automatically renew, unless auto-renew is turned off, until cancelled in the Manage Subscriptions section of your account settings. You will be charged no more than 24 hours prior to the start of the latest paid subscription period. To issue a refund or on any other issues with payments please contact our support team support@flo.health. Flo Health may, in its sole discretion, terminate your access to the HealthTap’s services, or any portion thereof, for any reason whatsoever without prior notice, in this case you may ask for refund or continue to use the mobile application subscription without telehealth services.
